Registration: Begins at 11:30am
Entry Fee: $45.00 per person - includes green fees, cart, prizes, and dinner
Dinner: Spaghetti dinner will be served at the golf course
Format: 2 Lady scramble - Both players tee off and then play best ball to the hole.
An optional skins game will be available for $10.00 per team.

I hereby waive, release, and discharge any and all claims for damages, death, personal injury, and/or property damage, which I may have, or which may subsequently occur to me as a result of my participation in this event. The release is intended to discharge Moose International Incorporated, any State or Provincial Moose association and/or any sponsoring Lodge/Chapter, their respective agent(s), and employee(s) from and against any and all liability arising out of, or connected in anyway with my participation in this event, even though liability may arise out of negligence or carelessness on the part of the person(s) or entity mentioned above. Terms hereof shall serve as a release and assumption of risk for my heirs, executors, successors, administrators, and assigns in exchange for my participation in this event.
